Chronic patients refer to people who live with health conditions that are somehow life threatening and they have to be monitored if they are to live comfortably. Nurse case managers help with the overseeing cases of patients who need long term care. In an attempt to fill their capacities, nurse case managers tend to work closely with the families of the patient and the patients themselves to provide the much needed care.
As may be the thought of many, nurse case managers are not only employed by hospitals but may be work for rehabilitatation centers, hospices or other facilities. It is no surprise to find that of every case that a nurse case manager has been employed to handle, she must have offered a different type of service since each patient has their own special need that are different from another. Due to the fact that every patient has needs that are to be attended to in a special way, a nurse case manager has to do her own research to understand how they will help a particular patient.
In caring for a chronic condition , we don’t expect a full recovery but progress needs to be assessed ,for this reason, a nurse case has to perform a follow up and evaluate the patient from time to time to find out if the patients situation is improving. A nurse case manager will spend a lot of time with the patient and when it comes to medication, they draw plans for medication.
In special cases , a case management nurse has to work like an insuarance representative to help with claims if there are any to be made , making the cost of care affordable is a prime responsibility of the nurse.
Since case management nurses are not directly involved in care but rather administrative , they need to be well organized Another skill that comes in handy if you are a case manager is good record keeping and file maintenance, a patient may have more than one file and having several patients means that one will have in their possessions several files and this calls for proper record keeping. A case management nurse needs to have the ability to multitask and to coordinate several aspect of the job especially if dealing with multiple patients at one time. Patients battling chronic conditions will be in a hospital for a long time and this makes it necessary for a case management nurse to be present so as to observe the care being given to the patient, here they can accept it or rule it as unfit for their patient. Not all patients are able to afford the cost of long term care and for this matter, a nurse case manager has to work with social workers to enable their patient access the care .
